57 yo female with a chief complaint of chronic migraine of greater then 30 years duration. Symptoms suggest multiple, mixed headaches with a clear cervicogenic component.  PMH is also positive for Meniere’s disease and right sided tinnitus.  Diagnostic Musculoskeletal Ultrasonography revealed a hypoechoic response over the deep multifidus and inter spinous ligaments at C23. Cold stress sympathetic skin response thermology showed an asymmetry pattern over the right upper cervical facet with infero-lateral radiation into the proximal trapezius.  Thermographic impression restated findings.
